Counseling Rates

$75-$100 per session

This rate may be covered in part or in full by your health insurance plan or Employee Assistance Plan (EAP). We ask that you review your insurance plan for following items:

• What is my deductible and has it been met?

  
  

• How many session per year does my health insurance plan cover?

  
  

• Do I need a referral from anyone prior to my first visit?

  
  

• Do I have a co-payment and if so, how much?

If you do not see your insurance plan listed here, please contact the office to verify. Please remember to check with your Employer for an Employee Assisted Program (EAP). You may be entitled to health benefits that are covered at no expense to you through this benefit.

Payment is expected at the time of your appointment.

Cancellation Policy:
If you need to cancel your scheduled appointment, we ask that you please notify the office as soon as possible. The more notice you provide us, may allow another individual the opportunity to scheduled an appointment. If you fail to attend your first appointment “intake/assessment” you will be referred to another agency or independent provider.

Report Rates
If you request that a report be made on your behalf (referral, recommendation, court, etc), you will be charged the rate it requires to complete the report. You will be responsible for the rate if rendered.

Court Rates
Often a report or attendance of your provider is required in some court cases. The rate for these services are covered in the informed consent form. You will be responsible for the rate if services are rendered.
